This advance care planning service is supported by SBRI Healthcare, an NHS England funded initiative. It is run by Digital Care Planning Limited, an independent company that was spun out of the Helix Centre* - part of the Institute of Global Health Innovation, Imperial College London. Digital Care Planning is based at St Mary's Hospital, Paddington, in London.
The company's mission is to get more people to make Advance Care Plans, thereby reducing unwanted and unnecessary hospitalisation at the end of life and ensuring healthcare is more aligned with patients’ wishes. We believe patients should have choices about the care they receive, and our tools are designed to ensure those choices can be easily recorded, discussed and shared with family, carers, and healthcare professionals.
Our service is designed to be used in England and Wales.
